Upon contact, parents receive an age-appropriate application package, including a questionnaire and forms for background and diagnostic information. Please complete and return these to the LRC to schedule an appointment.

Step 3

Assessments

The LRC specialists schedule an assessment for the child, in consultation with the parents/guardians. Based on the findings, the child may be referred for further assessments, services, or discharge. A Parent Conference Meeting follows the assessment, and parents receive a written report with findings and recommendations.

Specialized Services/Consultations

If parents and specialists agree, the child may be referred to specialized services or consultation in one or more areas at the LRC.
